VERY IMPORTANT ACTION STEP #1: Attached to this page you will find a practice “case history”. It consists of a DC Inventory, as well as an Office Layout Grid. The Inventory comes as a MS Word template that you download, complete and E-MAIL back to firstname.lastname@example.org. The Office Layout Grid is a PDF that you print, diagram your office layout and then fax back to (206) 322-0457.
SPECIAL FSM TIP: Please make copies of the Inventory for yourself and keep them handy for reference during your consulting calls. Normal turnaround time is 7 days.

VERY IMPORTANT ACTION STEP #2: Also attached is the Personality Test. This is an important coaching tool Drs. Lloyd and Birnbach use, so please complete it and also have your staff fill it out as well. You can fax it back to (206) 322-0457. Follow the instructions carefully.
VERY IMPORTANT ACTION STEP #3: Please provide a digital photo of you and your family and 10-15 digital photos of your clinic. These can be e-mailed to me email@example.com.

You will also receive weekly invitations to the FSM Webinars via email. Please make arrangements to attend them.
Finally, attached to this page is the 2014 FSM Seminar Schedule. Please mark each seminar on your calendar and make your travel arrangements now to prevent any conflicts. The FSM seminars are extremely important, as they will provide you with motivation and information, while allowing you to share with and learn from other Five Star clients.
